When: August 14th 5:00pm-7:00pm Where: MVU Main Lobby Followed By: Parent Meeting for Students of High School Athletes

Programs available to students are: 7th grade Girl’s Soccer, 7th grade Boy’s Soccer, 8th Grade Girl’s Soccer, Cross Country Running (7th & 8th grade combined) and Field Hockey  (7th & 8th grade combined)

***Please bring your insurance information and a copy of your last well child or sports physical from your doctors office. You must have had a well child or sports physical within the last two years on file with the school nurse to participate in the MVU sports program. Your doctor may also fax the info directly to our school nurse at fax # 868-5364.
